PRACTICE

Dashaun Justice is an intuitive counselor and certified Reiki practitioner. The information that he shares within a session is provided by intuition and the support of spirit. This work is not a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of your physician or other qualified healthcare provider with any questions you may have regarding a medical condition.

SESSIONS

Dashaun normally conducts an evaluation that can take place via email (phone conversations are also an option) to assess what you are looking for and if he is able to provide it. During this time, you can both decide if his services are the best fit for you to meet your goals.

Once you schedule your appointment, you will be expected to pay for it via the online platform provided to officially confirm your appointment. Your fee is non refundable unless we both agree that you were unable to attend due to circumstances beyond your control.

If you are unable to keep our appointment and inform me within the 24-hour cancellation period, you will not be charged additionally for that session and will be rescheduled at my earliest convenience.

LIMITS ON CONFIDENTIALITY

All Sessions are private. In most situations, information released about your sessions to others can only happen if you sign a written authorization form that meets certain legal requirements. These are situations that require you to provide written, advance consent.

Your payment for your session will serve as an acknowledgement and acceptance of the receipt of this Agreement and provides consent for those activities as follows:

We may occasionally find it helpful to consult other medical, spiritual and mental health professionals about a case. During a consultation, we make every effort to avoid revealing the identity of my client. The other professionals are also legally bound to keep the information confidential. Unless you wish it, we will not tell you about these consultations unless we feel that it is important to our work together.

At times we may employ administrative staff to help with scheduling and quality assurance practices. We may need to share protected information, such as your name and telephone number, with these individuals for purely administrative purposes.

Disclosures required to collect overdue fees are discussed elsewhere in the Agreement.
If a client threatens to harm themselves, we may be obligated to seek hospitalization for him/her or to contact family members or others who can provide protection.

If we have reason to believe that a child has been abused, the law requires that we file a report with the appropriate governmental agency, usually the Department of Family and Children’s Services.

If such a situation arises, we will make every effort to fully discuss it with you before taking any action and will limit my disclosure to what is necessary.

While this written summary of exceptions to confidentiality should prove helpful in informing you about potential problems, it is important that we discuss any questions or concerns that you may have now or in the future.

PROFESSIONAL FEES

Dashaun’s fees are for, but not limited to one-on-one sessions. Sliding scale fees are reserved via an honor system for those who vocalize in writing they are challenged economically and need these services.

EMAIL CONTACT

Due to Dashaun’s work schedule, he is often not immediately available by email. He does not check email when I am with a client.

BILLING AND PAYMENTS

You will be expected to pay for the sessions we agree to in full previous to session commencement and to hold your scheduled date. Payment schedules for other professional services will be agreed to when they are requested.

Singular Session appointments are transferable and non refundable.

Payments can be accepted via cash, major credit cards, bank transfer or e-check.

Dashaun will make every effort to return your email on the same day you place it. This may not always include weekends or holidays. If you have an emergency that cannot reasonably wait until the end of the business day, you are urged to call 911 if it is an emergency.
